If they're like dying or they can't do basic things I think we should still operate but also bill them afterwards and include the itemized bill, but if they've not actually got an emergency and they're holding up the ER for people who need it then no. We pay for our NHS through our taxes and if we want private then we pay for it, if I go to the US on holiday I'm not expecting free health care because I'm a UK citizen, I expect a bill because they bill people and they also expect insurance so I see no reason why we shouldn't treat any other person the same as they would treat us if we were in their country. I don't expect payment upfront or insurance, but I expect payment after the fact if they're from a country that charges for medical expenses.

I believe they should provide an EHIC like card to ensure their care however this should be an easily accessible thing for them

They should have some form of insurance document that permits them medical treatment that can be accessed online via a code on the individual's persons. That insurance would partially go towards the NHS and other emergency services.

I think depending on what kind of medical emergency it is should depend on whether or not foreigners should have to pay for emergency healthcare
